In the unfolding drama across our social platforms amidst armed conflict, the intentional weaponization of starvation casts a sinister shadow on the well-being of the besieged Palestinian population. As a disaster medic and firsthand witness dedicated to exposing the profound human consequences of intentional deprivation during wartime, a comprehensive exploration of the intricate physiological transformations is crucial to my ethical stance and professional duty. Within this expansive narrative, the challenges Palestinians face amid intentional starvation weave a story marked by anguish, resilience, and significant implications for public health.

We arrive at day 79 of the conflict and onslaught against Palestinians. We are clear that if medical aid workers were granted access, detailed accounts of the health conditions afflicting the Palestinian people would be meticulously documented. Numerous essays would be unfolding, offering deeper understandings. Here, I present an insightful perspective drawn from numerous instances in the history of disaster medicine, shedding light on what is likely occurring to the health of Palestinians amidst a starvation plot in occupied territories.

1. Systematic Malnutrition, Unfolding before our eyes.

Within the ongoing conflict, orchestrated starvation emerges as a malignant force, exacting its toll most harshly on the vulnerable Palestinian segments. The intentional withholding of sustenance plunges individuals into a dreadful realm of acute and chronic malnutrition, where the shadow of hunger haunts the most defenceless and the most vulnerable —children, pregnant women, and the elderly. The insidious lack of caloric intake becomes a silent aggressor, orchestrating a symphony of physiological degradation.

As the body contends with the stark absence of nourishment, a rapid and alarming breakdown of bodily tissues ensues. The skeletal frame of the vulnerable Palestinian population becomes a testament to the ravages of systemic malnutrition, a visual display of frailty and physical decline. In the throes of war-induced hunger, the very essence of life—nutrition—is weaponized, unravelling the fabric of health and normalcy.

2. Muscle Atrophy and the Erosion of Strength: A skeletal existence.

In the cruel calculus of war, where starvation is wielded as a strategic tool, the human body becomes a battleground. The absence of essential nutrients forces the body into a desperate bid for survival, cannibalizing its own muscle tissue for energy. The once robust and resilient musculature succumbs to the insatiable demands of systemic hunger, giving way to a pronounced display of muscle atrophy and wasting.

With each passing day of insufficient sustenance, the erosion of physical strength becomes palpable. The current weakened physical state of the Palestinian population renders them vulnerable to the conflict, diminishing not only their capacity for self-defence but also their bodily resilience in the face of a myriad of health complications. The skeletal fragility of bodies once animated by vitality serves as a stark reminder of the cruelty inflicted by wartime starvation.

3. Organ Dysfunction, A Symphony of Despair Unfolding.

As the shocking narrative of conflict-induced starvation unfolds, the consequences extend beyond the visible manifestations of physical decline. The deliberate deprivation of essential nutrients precipitates a cascading series of organ dysfunctions, transforming the afflicted body into a battleground where the vital organs bear the brunt of the siege.

The liver, kidneys, and heart—protectors of life's sustaining rhythms—succumb to the relentless assault of malnutrition. Their once harmonious orchestration falters, giving rise to discordant notes of dysfunction. The liver, tasked with metabolic equilibrium, becomes a struggling guardian, grappling with the repercussions of a nutrient-starved landscape. The kidneys, filters of life's essential fluids, strain under the weight of systemic dehydration. The heart, the tireless architect of the body's circulatory marvel, finds itself navigating treacherous waters as malnutrition undermines its once steadfast and strong rhythm.

4. Ketogenic Metabolism, A Desperate Dance with Survival in Progress.

In the tortuous corridors of prolonged starvation, the body of a besieged Palestinian in a desperate bid for survival, embarks on a transformative journey into the metabolic realm of ketosis. While ketosis is a natural adaptive response to fasting, its prolonged embrace within the ordeal of wartime deprivation takes on a sinister complexion.

Glycogen stores, the body's initial reservoir of energy, are depleted, prompting a shift towards the breakdown of adipose tissue for sustenance. The production of ketone bodies, once a physiological survival strategy, becomes an enduring testament to the body's relentless struggle for energy amidst scarcity. However, the protracted dance with ketosis takes its toll, pushing the boundaries of metabolic equilibrium and precipitating the danger of ketoacidosis—an extreme condition where the body's acid-base balance wavers on the brink. The worst outcome from this is bodily shock and eventual death. Prolonged painful few stages are endured before this outcome is reached.

5. Weakened Immune Response, The Fraying Fabric of Defence in Progress.

In the burden of war and famine, the body's defence mechanisms, once robust and vigilant, succumb to the erosion wrought by malnutrition. The intentional deprivation of essential nutrients becomes a saboteur of the immune system, leaving victims defenceless against the onslaught of infectious diseases that propagate with ruthless efficiency.

Children, in their innocence, become the unwitting casualties of a weakened immune response, bearing the disproportionate burden of preventable illnesses. Respiratory infections and diarrheal diseases sweep through vulnerable communities with a merciless intensity, exploiting the chinks in the armour of immunity compromised by malnutrition. The humanitarian crisis deepens as the silent casualties of conflict—those claimed not by weapons but by the opportunistic infections of famine—underscore the magnitude of the health catastrophe.

6. Dehydration and Electrolyte Imbalances, A Parched Landscape Unfolding.

As the harrowing saga of intentional starvation unfolds, the absence of clean water compounds the tragedy, transforming the body into a dehydrated landscape. Dehydration, an insidious companion to malnutrition, descends with rapidity, its effects booming through the delicate balance of electrolytes—the body's conductors of life's essential currents.

The lack of proper hydration intensifies the severity of health conditions, hastening the deterioration of physical and cognitive functions. Electrolyte imbalances, once guardians of homeostasis, become forerunners of crisis, manifesting in symptoms ranging from weakness and confusion to life-threatening arrhythmias. The dehydrated landscape mirrors the parched desolation wrought by conflict, where the scarcity of sustenance extends beyond the physical to the very essence of vitality.

7. Cognitive Decline and the Shattered Tapestry of Mental Health Unfolding.

In the protracted shadows of starvation, cognitive function becomes a casualty of war for the affected population, with individuals navigating a treacherous terrain of mental decline. The once vibrant tapestry of cognition unstitches, as weakness, fatigue, and the ceaseless pangs of hunger cast a shroud over the mental landscape.

The mental health consequences of conflict-induced starvation are profound, with Palestinians grappling not only with the physiological burdens of malnutrition but also with the invisible scars etched upon the psyche. Depression, anxiety, and post-traumatic stress disorder (PTSD) become silent adversaries, further complicating the already intricate tapestry of suffering woven by the intersection of war and deprivation.

8. High Mortality Rates, The Tragic Finale Developing.

As the symphony of suffering reaches its climax, the synergistic impact of war-induced starvation culminates in elevated mortality rates for the Palestinian population—a heart-wrenching conclusion to a narrative of anguish. The most vulnerable among the afflicted—Palestinian children, pregnant women, and the elderly—bear the disproportionate burden of death, their frail bodies succumbing to the relentless assault on health and vitality.

Children, in particular, face an increased risk of mortality, their diminished resilience unable to withstand the union of malnutrition, infectious diseases, and the trauma of conflict. The spectre of preventable death looms large, underscoring the urgency of humanitarian interventions to alleviate the suffering wrought by the intentional weaponization of hunger against the Palestinian population.

As a disaster medic chronicling the convergence of war and its impact on the overall physical and mental well-being of the Palestinians, it is not only crucial to bring this narrative to the forefront but also an ethical obligation. The accounts of Palestinians trapped in the web of conflict-induced starvation transcend mere statistics; they are human stories etched with pain, resilience, and an indomitable spirit that persists in the darkest of hours. The call to action resonates, urging the world to witness the silent yet resounding catastrophe unfolding and to tirelessly advocate for the essential principles of humanitarian aid, peace, and the restoration of the basic human rights to sustenance and dignity for the Palestinian people.
